		Forms We’ve listed many of our applications, service enrollment forms, source documents and other forms on this page for your convenience. All of these forms are PDF files that can be viewed using Adobe Acrobat Reader. If you don’t already have Acrobat on your computer, you can download it for free by clicking here. Instructions Click once on the name of the form that you need. When you do this, Acrobat will automatically open and an image of the form will appear in your window. Print the form. Complete the form by hand writing your information using black or blue ink. Sign your name (and have joint owners sign if necessary). Be sure to keep a copy of your signed form for your records. Mail your form to: Vantage Credit Union, P.O. Box 4433, Bridgeton, MO 63044. Or, you can bring the completed form to any of our branch locations. Please do not e-mail the form to us because we require pen-based signature(s) to process your request. List of Vantage Credit Union forms Account Card: Use this form to establish your Vantage Credit Union membership, request to open a Vantage Credit Union checking account, request to open a money market account, assign/change beneficiaries, or to assign/change joint owners. Alumni Club Membership Request Form: Use this form to sign up for Vantage Credit Union’s Alumni Club. If you’re age 50 or older and maintain an aggregate Vantage Credit Union deposit balance of $5,000 or more, you’re eligible for Alumni Club membership and its special benefits. Change of Address Form: If you are moving, please use this form to notify us about your new address. Check Reorder Form: Use this form to reorder personal Vantage Credit Union checks. For faster service, you can instead reorder your checks online via secure Internet connection with our check vendor. Cross-Account Transfer Form: Use this form to give yourself the ability to transfer money from one account number to a different account number. Deposit/Transfer/Withdrawal Form: This is the same form we have in our branches for making deposits, transfers or withdrawals at a teller window. We’ve included this form here because many members enjoy the convenience of mailing in their deposits. Electronic Services Agreement: Use this form to sign up for Vantage Credit Union Online (our free Internet banking service), request EZ Pay bill payer service, request a free Vantage Credit Union Visa Check Card or ATM Card, request Touch Tone Teller (banking by phone) authorization code, or request a new PIN for your existing Vantage Credit Union Visa Check Card or ATM Card. Express Transfer Authorization Agreement: Use this form to request our Express Transfer Service. Fee Schedule: Lists fees charged for certain Vantage services. MasterCard Automatic Payment: Use this form to request automatic payment for your Vantage MasterCard credit card. Membership and Account Agreement: This agreement is the contract of deposit which covers your and our rights and responsibilities concerning Vantage membership and account(s). Quest For Education Scholarship Application: If you’re a graduating high school senior and are a Vantage Credit Union member, you’re eligible to apply for one of twenty-five $1,500 college scholarships we award each April. This year's application deadline is Friday, December 5, 2008. Judging will take place from December, 2008 through February, 2009 and winners will be notified by Friday, March 6, 2009. Secret Savers Account Card: Use this form to establish Vantage Credit Union membership, and Secret Savers Club membership, for your child age 10 and under. Visa Automatic Payment: Use this form to request automatic payment for your Vantage Visa credit card. Top of Page
